Instructions for E-mailing Electronic Order Form

 

1. On “Request Form” page:

A. Open electronic order form.

B. Box appears, “Do you want to open or save file?”

OPTION 1 (Save Form)

Click “Save”

  • Select location to save document (Desk Top, My Documents, etc.)

  • Add your last name, initials or first name in front of “Electronic-Order-Form”.
    Expl: Jones, TJ Electronic-Order-Form.doc.

  • Click “Save”

Open Microsoft – WORD

  • Find and Open electronic order form saved above.

  • Verify that document opened is the same one saved with your name and initials or first name.

  • Complete form.

  • Click “Save”.

OPTION 2 (Open Form)

  • Click “Open”

  • Complete the form by filling in boxes.

  • Click “Back” arrow on monitor screen being worked on.

  • A pop up screen appears with the following message:
    “THIS DOCUMENT HAS BEEN MODIFIED. DO YOU WANT TO SAVE CHANGES.”

  • Option are: YES NO CANCEL

  • Click "Yes"

  • In the “file name” line, add your last name, initials or first name in front of “electronic order form”.

  • Open location to save document (Desk Top, My Documents, etc.)

  • Click “save”

  • The screen goes away.

For either case above:

Go to internet screen with “santaforstarlight.com” website on “Request Form” page.

  • Click on “head.elf” which opens an email to submit the order form.

  • Attach the saved, completed electronic order form with your name to the email.

  • Complete email; subject and any comments to add to the order.

  • “Send” email.
