1.      On “Request Form” page:

 

a.       Open order form with an ADOBE READER (See Request Form page for Adobe connection).

 

b.      The Order Form can be filled in from your computer.

 

i.        IMPORTANT – Click “Save As” AND change the file name by adding your name in front.

EXAMPLE – “Jay Jones 11 Form4A Web Order Form 11.25.11”

 

ii.      If you choice to hand fill in the Santa order form.

1)      Mail, Fax or Scan the document and submit as noted on the form

2)      COPY blank & complete in Black Ink.
